A planned follow-up review applies to scheduled reviews for an established chronic condition, including medication adjustment, monitoring of treatment response or review of previously agreed management plans. Patients returning with a new or unrelated concern, or requiring a comprehensive reassessment, will be booked as a new initial consultation.
